Ladbroke Grove Rail Enquiry
Provides information from the public inquiry under Lord Cullen into the fatal rail crash at Ladbroke Grove on 5 October 1999. The joint HSE investigation with the British Transport Police has been completed. The investigation identified a number of significant factors whereby the signalling in the Paddington area did not comply with relevant industry standards. Signal sighting experts, came to the overall conclusion that viewing conditions presented an exceptionally difficult signal reading task.
