- Provides information from the public inquiry under Lord Cullen into the fatal rail crash at Ladbroke Grove on 5 October 1999. The joint HSE investigation with the British Transport Police has been completed. The investigation identified a number of significant factors whereby the signalling in the Paddington area did not comply with relevant industry standards.
